WHAT TO WRITE IN NOTEBOOK:
BE SURE TO DATE EVERY ENTRY.
NAME/POSITION of person you are seeing (internist, pharmacist, surgeon, etc.) of any person who tells you to do something (take this med, do this, don't do that, etc.)
WHAT THEY TELL YOU TO DO
ANY UNUSUAL SYMPTOMS OR REACTIONS TO MEDS. ETC.
DATE YOU START MEDICATION/DATE ENDED
BE SURE TO DATE EVERY ENTRY. THEY WILL ASK YOU HOW MANY DAYS AGO SUCH AND SUCH HAPPENED...
Good Luck!

I second this. You've got to be proactive. When I was going through chemo/rad I followed a similiar routine, called the doctor out on a few things right away, and gained everyone's respect and IMO got better care for it.
