SPOILER You need to select your team correctly. I sent legion through the pipes, morden back with the crew and samara to do the biotic shield. I had legion and jack with me on the final run. I always picked Garrus to lead the second team. If you dont have your ship fully upgraded, then you will lose 1 character for each upgrade you didn't do. Characters are also far less likely to make it through if they aren't loyal, however they can still make it as long as you have the ship upgraded and make the correct team choices.
