How does one accidentally overdose on prescription pills? Does one think, "Oh, a handful isn't enough. Tonight I go for two handfuls!"

An "accidental overdose" can also mean two or more prescriptions mixed in the body and had a lethal effect, when separately they should have been fine. A patient who discloses all of the drugs he is taking, prescription or otherwise, will raise a red flag for a pharmacist who sees that new drug X will be dangerous when taken with drugs Y and Z. I doubt he's that forthcoming and on the record, hence the "accidental overdose."
