Most of people who fall aslkeep with lit cigarettes are hardcore drunks.

If you're both a drinker AND a smoker, there's a good chance that you'll smoke more after getting hammered. So you're smoking, you pass out and the cigarette falls. It happened to me once but the lit cigarette burned my skin and i woke up.

It's a classic death for hardcore alcoholics who live alone or junkies.
