Some would say that the fair way to do it is to establish one single pool that everyone has equal access to. Or, go to a straight cash fee-for-service system. If you can't pay for it, you can't have it. It would be unfair if someone got treatmet on credit and then defaulted. Cash up front. Is that better?

But, of course, there's the question of who deserves health care as compensation. Our system would be fairer, I suppose, in a merit-based compensation structure, but that's not what we have.
