You don't pee out of the same hole as where the tampon goes. You're not going to pee sideways due to a tampon being inside of you.

A lot of people don't change whatever is down there every time they pee but I think it's better to do so for cleanliness reasons. Depending on your flow.

edit: and I loved this episode, as per usual
